We engineered a Remote Patient Monitoring (RPM), which allows pharmacists to track vital health metrics such as blood pressure, glucose levels, and heart rate remotely using monitoring devices. Automated alerts ensure that any significant changes in a patient’s health are promptly addressed, reducing the need for frequent in-person visits and minimizing the risk of complications. Additionally, our Medication Therapy Management (MTM) feature enhances patient adherence by enabling pharmacists to create and monitor personalized medication plans, while Transitional Care Management (TCM) feature ensures smooth transitions between different care settings through pharmacist-coordinated support and comprehensive follow-up care.
Ensuring the security and confidentiality of private patient data is important, particularly in compliance with HIPAA in the USA. To address this, we implemented strict security measures to protect sensitive patient information from unauthorized access, breaches, and cyber threats. This includes encryption of data both in transit and at rest, as well as access controls. Additionally, to test and develop our systems without compromising real patient data, we utilize synthetic (anonymized) data sets. This helps validate the functionality and security while safeguarding actual patient information.
